Is it mandatory to mention person name for Qurbani?

                          Till today I know that it is mandatory to mention person name for Qurbani.

Just got a pamphlet from a Masjid it says, 

> According to the Quran and Sunnah it is not necessary to mention you
> name for  the sacrifice.

What is the proof?

And what about if I want to sacrifice for my grand parent, means for dead people?
